A groundbreaking geriatric care health program, funded by the Friends of St Monica’s Hospital (FOSMH), is already benefiting elderly patients in the area.
The Proactive Health and Care Clinic, also known as the Frailty Project, provides a comprehensive assessment of the needs of elderly people, and provides help, advice, and support tailored to individuals.
Health professionals, including a GP, physiotherapist, occupational therapist, and a social prescriber, are carrying out assessments at St Monica’s Hospital. Patients are referred to this geriatric care program from Millfield, Tollerton, and Stillington surgeries. The spectrum of issues that patients might face include reduced mobility, falls, loneliness, strain on carers, or prescription-related issues.
Dr John Morris, who is leading the project, has said that the program stands to maintain people’s health, wellbeing and independence. Figures from August show that it is already having a beneficial impact. The month saw 22 patients assessed, who were experiencing challenges related to memory, fatigue, bladder problems, falls, postural-hypotension, pain, and nutrition.
More than 30 unnecessary drugs were stopped, cutting NHS costs and a range of support was identified.
